1. Select your Residence Card^*, if applicable, to confirm your "Status" of residence and "Period of Stay". If your remaining "Period of Stay" is less than 3 months, please renew your Residence Card before you apply and upload both old and renewed Residence Card images.
2. If you are a foreign national who have been in Japan for LESS than 6 months, one of the following supplementary documents is also required.
   • Employee ID Card showing that your office is located in Japan
   • Health Insurance Card which has the name of your employer on it
   • Certificate of enrollment stamped/signed by your employer, which include: your full name, company name, company address, and the start date.
